NOMINATE A WOUNDED VETERAN TO RECEIVE A FREE ACTION TRACKCHAIR

Anyone can nominate a veteran in need of assistance with mobility by submitting the serviceman or woman’s story of sacrifice, strength and courage to Honor Heroes. Selection of the winning veterans will be announced after a certified group reviews the materials submitted to select the deserving warriors who will receive Action Trackchairs. Chief Guitar Foundation raises funds to cover the costs of the Action Trackchairs.

Entry Requirements

Each Entry:

  • Must be in English
  • Must include an essay of 500 words, typed double-spaced with 1” margins or less; or a two-minute video that details the above-stated information
  • Must be the original work of the entrant
  • Must be signed by the nominee

Nomination Rules

All entries must be received on or before midnight December 31, 2018.

During the Contest Period, an eligible entrant may visit chiefguitarfoundation.org (“Website”), enter his/her email address, and register by entering all information requested on the registration form including, but not limited to, complete first and last name (no initials), address (no P.O. boxes), city, state, ZIP code, phone number, date of birth, and confirmation signature that he/she has read and agrees to be bound by these Official Rules.

The entrant may nominate himself/herself or an eligible friend (“Nominee”) by following the instructions to submit either (1) if entrant is nominating himself/herself, entrant’s branch of service; an essay describing the sacrifices, including when the Nominee was wounded. If possible, please include a photo) or (2) if entrant is nominating a Nominee, the Nominee’s first and last name, branch of service, email address, city, and state; an essay describing the sacrifices, including injuries incurred during combat and a photo (optional). PLEASE NOTE: If an entrant submits a Nominee other than himself/herself, the entrant must obtain permission from the Nominee prior to submitting the Nominee’s name or other information and photo (if a photo is submitted).

NOTE: If any entry fails to comply with any of these Entry Requirements or any other provisions of these Official Rules, Sponsor and Administrator reserve the right, but are not obligated, to disqualify the entrant.

By submitting an entry, an entrant agrees that his/her entry is gratuitous and made without restriction and will not place Sponsor under any obligation because the entry is submitted on a non-confidential basis without any additional compensation whatsoever to the entrant. Entrant agrees that Sponsor may post the entry on any websites associated with the Contest, including Sponsor’s social media channels.

Submitting an entry constitutes entrant’s consent to give Sponsor, where applicable, and its designees, licensees, successors and assigns (collectively, “Licensed Parties”) a royalty-free, irrevocable, perpetual, non-exclusive license to use, reproduce, modify, publish or create derivative works from and display the entry in whole or in part, on a worldwide basis, and to incorporate it into other works, in any form, media or technology now known or later developed, including for promotional or marketing purposes, and that Sponsor may use entrant’s name, user name(s), biographical information, and/or likenesses (including, without limitation, profile photo(s)) in connection with the uses described herein. If requested, entrant will sign any documentation that may be required for Licensed Parties to make use of the nonexclusive rights entrant is granting to use the entry. This license expressly includes a right (but not the obligation) for Sponsor to modify entries to remove any third party intellectual property or to remove entries in their entirety.

Judging scores will not be revealed.
